The extension is certainly added as per the screenshots
It appears twice but neither open.
I tried uninstalling and reinstalling, but got the same result.
Its on Silverblue and GNOME Web is v46.1 (Flatpak)
Also, I seem to remember a list of tested extension being circulated when WebExtensions appeared in v45. Does that still exist?
Videos: audio works but no video.
Is even ready for prime time?

For WebExtensions Not at all. Thats why its a hidden developer setting.
Bitwarden is a complex actively developed extension. It worked for me in the past but I’m not surprised if a new failure happened.
Someone familiar with extensions would have to look into it.

I see they said it is low on the priority list, so I will pass for now.
